Wegovy alternatives in 2026: every real option, compared

October 9, 20267 min

Most people looking for a Wegovy alternative are not unhappy with the idea of a GLP-1 medicine. They are unhappy with something around it: the price, an insurance denial, a pharmacy that keeps running out, or nausea that made the first month miserable. The good news is that in 2026 there are more real options than there have ever been, including two pills. The honest news is that they are not all equal, and the right one depends on which of those problems you are trying to solve. Here is every option worth knowing about, with what each one actually did in its main clinical trial.

First, what Wegovy is

Wegovy is Novo Nordisk's brand of semaglutide, approved by the FDA for chronic weight management. It is a once-a-week injection, and since December 2025 there is also a daily Wegovy pill. In the STEP 1 trial, adults on the weekly injection lost 14.9% of their body weight on average over 68 weeks, compared with 2.4% on placebo. That number is the benchmark everything below gets measured against.

Zepbound (tirzepatide): the one that lost more weight

Zepbound is Eli Lilly's brand of tirzepatide, also a weekly injection. It works on two hunger hormones (GLP-1 and GIP) instead of one. In SURMOUNT-1, adults on the highest dose lost 20.9% of their body weight on average over 72 weeks. More usefully, SURMOUNT-5 put the two medicines head to head: 20.2% average weight loss on tirzepatide versus 13.7% on semaglutide. If your main question is which medicine works best, this is the trial that answers it. Our semaglutide vs. tirzepatide guide goes deeper.

The two GLP-1 pills

If needles are the problem, 2026 changed things. The Wegovy pill (oral semaglutide, 25 mg) was approved in December 2025; in its OASIS 4 trial, adults lost 13.6% of their body weight on average over 64 weeks. It has to be taken first thing in the morning on an empty stomach with a little water, then nothing else for 30 minutes. Foundayo (orforglipron), Eli Lilly's pill, was approved in April 2026; on the top dose in its ATTAIN-1 trial, adults lost 12.4% on average over 72 weeks, and it can be taken any time of day with or without food. Both pills landed a little below the injections in their trials. Our Foundayo vs the Wegovy pill comparison goes through the food rules, side-effect counts, interactions and heart data line by line.

Older prescription options

  • Liraglutide (Saxenda): an earlier GLP-1, injected daily rather than weekly. In the SCALE trial it produced 8.0% average weight loss over 56 weeks. Generic versions are now sold in the US.
  • Phentermine-topiramate (Qsymia) and naltrexone-bupropion (Contrave): daily pills that work on appetite through different pathways. They are not GLP-1 medicines, and their average results in trials were well below Wegovy's.
  • Phentermine on its own: an older appetite suppressant usually prescribed for short periods only.

Compounded semaglutide and tirzepatide

Many telehealth programs, including ours, prescribe compounded semaglutide or tirzepatide. These are prepared by a licensed compounding pharmacy for an individual patient's prescription. They are not FDA-approved, they are not Wegovy or Zepbound, and they are not generic versions of either; the FDA does not review compounded medicines for safety, effectiveness or quality before they are dispensed. The trial results above were for the brand-name medicines. Whether a compounded medicine is appropriate for you is a licensed provider's decision, and the questions worth asking are who compounds it and under what license. Our guides on whether compounded GLP-1 is legit and the 2026 rules on compounding cover that in detail.

What does not count as an alternative

Supplements sold as a natural Ozempic, GLP-1 boosters or berberine are not GLP-1 medicines and have nothing close to the evidence above. Products sold online as semaglutide without a prescription are a separate, real danger; our grey-market GLP-1 guide explains why. And there is no over-the-counter GLP-1 at all, which we cover in is there an over-the-counter alternative to Wegovy.

How to choose

Whichever way you lean, a licensed provider has to decide what is safe for you. Your history, other medicines and conditions such as a personal or family history of certain thyroid cancers rule some of these out.
